No, spotting and cramping aren't an indication of the pill's effectiveness: they're just side effects that happen sometimes with hormonal methods, and it doesn't mean that anything is wrong. If you have been taking your pills as directed, then you can assume that you are fully protected.

No, this wouldn't be a condom failure: the condom did not slip off inside of you or break in a way that let any fluids get near your genitals, so it did its job.

Condoms can be a bit slippery, so it's possible this might happen again, but even if it does it's not a cause for concern.
