I'm probably reading this wrong but do you mean it almost always occurs when there is intercourse but it can also occur through very direct genital contact and or when someone ejacualtes directly inside a vagina or onto the Vulva.

Or do you mean it's usually by these methods but there can be other ways that it occurs?
