Condoms?

I have some stupid questions about condoms that I have been wondering about and hope you can help.

If a guy has precum on him and one of us touches it with our hands and wipe off so it is dry, is it possible that precum can be transferred from hands to the condom while putting the condom on? Would that pose a risk for pregnancy?

How tight is too tight for a condom?

How does a condom break? Would going too fast break down a condom? What if you go slow for a couple minutes and thats it?
