What do doctors and health sites mean when they say something is highly unlikely
Does it mean almost 0% chance of that "something" happening?

We can't answer for what other people mean when they use that phrase, but it means what it says: something is very, very unlikely, but not impossible.
And you to whom adversity has dealt the final blow/with smiling bastards lying to you everywhere you go/turn to and put out all your strength of arm and heart and brain/and like the Mary Ellen Carter rise again.
